The Cedar Health Alliance Plan
What is Cedar Health Alliance?
Cedar Health Alliance is a health plan created for churches and faith-based organizations. It combines traditional medical coverage with virtual care, care navigation, and direct-pay healthcare options designed to give members more ways to access care while helping participating organizations manage healthcare costs.
Is Cedar Health Alliance self-funded?
Yes. CHA uses a self-funded model in which participating organizations share risk across the larger plan. Organizations still pay a predictable monthly amount, while the plan is structured to provide greater visibility into healthcare spending and protection against unusually large claims.
What plan options are available?
CHA offers multiple medical plan options with different deductibles and out-of-pocket costs. Each includes access to Amaze Health for care navigation and virtual care, along with opportunities to access certain services at no cost through the plan’s direct-care options.
Getting Care
Where do members start when they need care?
Amaze Health is the primary care-navigation resource for CHA members. Members can use Amaze for virtual primary and urgent care, mental health support, and help navigating other healthcare needs.
Can members still see doctors and specialists in person?
Yes. CHA includes coverage for in-person primary care, specialists, hospitals, and other medical services. Member costs depend on the plan and how care is accessed.
Do members have to use Amaze for every appointment?
Amaze helps members understand their options and can coordinate care, particularly when members want to access the plan’s no-cost care pathways. Traditional in-person care remains available under the medical plan.
What is Fair Market Health?
Fair Market Health gives members access to direct-pay healthcare services such as labs, imaging, therapies, and certain procedures. Eligible services may be available at no cost when coordinated through Amaze and Fair Market Health.
Benefits & Prescriptions
What care is available at no cost?
Amaze Health provides no-cost virtual care and care navigation. Certain labs, diagnostic testing, imaging, procedures, and other services may also be available at no cost when coordinated through Amaze and Fair Market Health. Exact benefits depend on the plan selected.
How are prescriptions covered?
CHA includes prescription coverage with different cost levels depending on the medication. Generic medications included in Tier 1 are available at no cost, while other medications may have copays or require additional review.
Is mental health care included?
Yes. Mental health therapy is available through Amaze Health at no cost, with additional coverage available according to the member’s medical plan.
What happens in an emergency?
Members should seek emergency care immediately. Emergency services are covered according to the member’s medical plan.
Enrollment & Support
Who helps employees understand and enroll in their benefits?
CHA uses a dedicated enrollment team to help employees understand their options, choose a plan, complete enrollment, and confirm provider participation.
What if I receive a balance bill?
Members should contact Amaze Health or AMPS for assistance with an out-of-network balance bill.
